NGPG, which has been using Phytel's registry and ™ solution for some time, will add the and applications this summer. Phytel Insight enables healthcare organizations to identify patient care gaps and to measure their performance in population health management, and Phytel Coordinate delivers patient interventions in support of care management. The (NCQA) allows physician practices that use these and other Phytel solutions to automatically meet certain NCQA requirements for recognition as patient-centered medical homes.
NGPG realized that, in order to scale up the requisite functions quickly, it would need a proven solution that could be interfaced with its ambulatory-care electronic health record. The group was looking for a best-of-breed population health management solution, and with Phytel's domain expertise it was able to meet all its requirements.
