Media Advisory/Flight Safety Investigation Report: CH-147 Chinook Incident AUGUST 5, 2010

(firmenpresse) - OTTAWA, ONTARIO -- (Marketwire) -- 12/18/12 -- The final Flight Safety Investigation Report regarding the CH-147D Chinook in-flight explosion and fire which occurred August 5, 2010 during a sustainment mission outside Kandahar Airfield is now available on the Air Force Directorate of Flight Safety website.

The aircraft was on a sustainment mission outside Kandahar Airfield, when it experienced an in-flight explosion and fire. The occurrence took place while the aircraft was flying at low altitude from the forward operating base Masum Ghar to the Panjwai District Centre. The pilots were able to successfully land the aircraft in an open field, after which all aircrew members and passengers exited the aircraft. Some aircrew and passengers sustained minor injuries while egressing through the emergency exits. After the landing and egress, the aircraft was subsequently destroyed by the fire.

The scope of this Flight Safety Investigation was limited to the review and analysis of the effectiveness of aviation life support equipment and egress procedures, as well as other issues pertaining to occupant safety. The cause of the explosion and fire, vulnerability to insurgent threats, operational tactics and countermeasures were not within the scope of this Flight Safety Investigation Report.

The Flight Safety report identifies the requirement for cabin and passenger safety policy and cabin safety standards for both legacy and new fleets.

The flight safety investigators have made several recommendations that will now be carefully considered by Air Force officials, who will take appropriate action with the aim of preventing injury in the future.
