This award was presented on July 18, 2012 and is achieved only when a provider demonstrates a high level of quality in all their business operations and services. Forum earned the three-year accreditation after completing a comprehensive review process that included meeting quality standards and a commitment to quality operational practices and procedures in the areas of financial stability, human resource training, patient care, delivery services, and billing practices. Accreditation by the Healthcare Quality Association on Accreditation assures Medicare and other health payers that the highest levels of quality are practiced within all areas of the company.
