According to a 2012 report by the Centers for Medicare and Medicaid Services (CMS), 30-day hospital readmissions increase with the number of chronic conditions, and depression ranks in the top eight at 14 percent. For disabled beneficiaries less than 65 years of age, depression is 2.3 times more likely.
MyHealthTone enables simple and efficient online screening for symptoms of depression.
